Instructions

1. Download the Expense Report Form (PDF format). For expenses incurred prior to June 30, 2008, please use this form.                      

2. Print and complete the form according to the directions. 

3. Mail to:

   Seattle, WA 98101-2539

4. PLEASE NOTE: This form will be updated whenever mileage rates change. Please obtain the most recent copy to ensure correct expense reimbursement.
